2. Medical Services Disclaimer

RASMED Clinic provides in-person medical care and patient portal services. The information provided on our website is for general informational purposes only and does not constitute medical advice. It is not a substitute for professional medical diagnosis, treatment, or advice from a qualified healthcare provider. Always seek the advice of your physician or other qualified health provider with any questions you may have regarding a medical condition.

In the event of a medical emergency, call 911 or go to the nearest emergency room immediately. Do not use our website, portal, or messaging system for emergencies.

3. Patient Portal Account

3.1 Account Creation

To access our patient portal, you must create an account and provide accurate, current, and complete information. You are responsible for maintaining the confidentiality of your login credentials and for all activity that occurs under your account.

3.2 Account Security

You must notify us immediately at 718-859-7446 or admin@rasmednyc.com if you suspect unauthorized use of your account. We are not liable for any loss or damage arising from your failure to maintain the security of your account.

3.3 Account Termination

We reserve the right to suspend or terminate your account and access to the portal if you violate these Terms, provide false information, or engage in conduct that is harmful to the Clinic, its staff, or other patients.

4. Appointment Policy

• Scheduling: Appointments may be scheduled online, by phone, or through our patient portal.
• Cancellations: Please cancel or reschedule appointments at least 24 hours in advance. Repeated no-shows may result in discharge from the practice.
• Arrival: Please arrive 15 minutes before your scheduled appointment to complete any necessary paperwork.
• Late Arrivals: Patients arriving more than 15 minutes late may need to reschedule to avoid disruption to other patients' care.

5. Payment and Insurance

• Co-payments and Deductibles: All co-payments, co-insurance, and deductibles are due at the time of service.
• Insurance Verification: It is your responsibility to verify that RASMED Clinic is in-network with your insurance plan prior to your appointment.
• Self-Pay: Patients without insurance are required to pay for services at the time of the visit. Payment plans may be available upon request.
• Billing Disputes: Please contact our billing department within 30 days of receiving your statement with any questions or disputes.

6. Patient Responsibilities

As a patient, you agree to:

• Provide accurate and complete medical history and information
• Follow the treatment plan agreed upon with your provider
• Notify us of any changes to your insurance, contact information, or medical status
• Treat all clinical and administrative staff with respect and courtesy
• Use the patient portal only for its intended healthcare communication purposes

These Terms are governed by the laws of the State of New York, without regard to its conflict of law principles. Any disputes shall be resolved in the state or federal courts located in Kings County (Brooklyn), New York.
